Amoxapine is a tetracyclic antidepressant which is related to the dibenzoxazepine family even though it is categorized as a secondary amine tricyclic antidepressant. It has several mechanisms of actions in the brain. Mainly it acts as a serotonin and norepinephrine reuptake inhibitor. It also acts as H1, a1 adrenergic and dopaminergic receptor antagonist. Due to the wide array of pharmacological actions of Amoxapine, it may cause serious adverse effects at times. That is why this has been a very serious drug in medical practice.

Amoxapine is also known by its generic names such as Asendin, Asendis, Defanyl, Demolox and others. But, whatever the generic name it is, this drug can be bought only with a prescription. This atypical antipsychotic is used to treat schizophrenia, major depressive disorder, psychosis in Parkinson’s disease. Amoxapine is administered orally in the form of tablets. With a bioavailability of more than 60% and a protein binding capacity of 90%, this drug is metabolized in the liver and excreted by the kidneys.

Usual effective dose for an adult is 200-300mg per day and is normally taken as a single dose at bedtime. The higher doses are given as several doses throughout the day and not at once. It is toxic in overdose, which will lead to renal failure, rhabdomyolysis, coma, seizure and even status epilepticus. Therefore, in case of overdose, rush into the nearest hospital for further treatment. In case of missed dose, you shouldn’t add up the missed dose to the next one. Leave it and take the next dose when it’s the right time.

There are some serious side effects which may be caused by Amoxapine and they are; changes in cardiovascular system (hyper-hypo-tension, tachycardia, arrhythmia and etc.), agranulocytosis, leukopenia, neuroleptic malignagt syndrome, tardive dyskinesia, thrombocytopenia, jaundice, liver failure and paralytic ileus. Other side effects may be allergic reaction, nausea, vomiting, syncope, mydriasis, itching, diarrhea, menstruation irregularity and others.

This drug is contraindicated for patients with severe cardiovascular diseases, uncorrected narrow angle glaucoma, post myocardial infarction and whoever has allergic reaction to this drug. This drug is also not recommended for the lactating mothers.

It is important to mention if you have taken any MAO inhibitors such as isocarboxazid (Marplan), phenelzine (Nardil), selegiline (Eldepryl, Emsam, Zelapar), tranylcypromine (Parnate) and others within 14 days or whether you are taking it at the moment. Actually mention all the drugs you may be taking, even the vitamins and supplements before your doctor prescribe you this drug. Do not take alcohol with these tablets as it may make you drowsy.
