Clompiramine is a tricyclic antidepressant which is also trading as Anafranil or Clofranil. It mainly acts as a highly selective serotonin reuptake inhibitor while its other mechanisms such as antagonism of H1 receptors, muscarinic receptors and a1 receptors leads to the adverse effects of this drug. This drug is considered to be an essential medication in the general health system and thus, it is listed in the world health organization’s list of Essential Medicines.
Clompiramine shows 50% bioavailability and a 97% protein binding capacity. It is metabolized in the liver and excreted by the kidneys with the half life of 32 hours in the blood.
This drug is used to treat Obsessive compulsive disorder, Major depressive disorder, Panic disorder, Body dimorphic disorder, cataplexy together with narcolepsy, Premature ejaculation, Depersonalization disorder, Chronic pain, Enuresis and Trichotillomania. It is administered by the oral route and in some cases also can be used intravenously. Starting from a lower dose such as 25mg per day, it will be gradually increased according to the patient’s response towards the drug. After two weeks of the time the dose can be increased even up to 250mg in total, but, it is necessary to give this in several doses per day. This tablet is better tolerated when it is administered with a meal. In case you missed a dose, leave it if it is the time for the next dose; do not try to add up to two doses together to compensate the missed one.
It is important to keep your bottle of tablets is a cool, dry place without letting it expose to direct sunlight or heat and also far away from children.
There are several side effects which may bother you while you are on this drug; They are dry mouth, constipations, blurred vision, dizziness, headache, feeling faint or light headedness, diarrhea, ECG changes, sweating, hot flushes, difficulty urination, skin rashes, increased appetite, insomnia, muscle weakness, changes in sexual function, changes in taste and hearing.
Overdose of Clomipramine may lead to signs of CNS depression; such as stupor, coma, drowsiness, ataxia, restlessness, mydriasis, convulsions, muscle rigidity, cardiovascular changes such as arrhythmia, tachycardia, hypotension, shock, heart failure or cardiac arrest and also respiratory depression, apnoea, cyanosis, anuria and others. There is no specific antidote for the overdose, but, activated charcoal is used to absorb the drug in the stomach. It is important to take the overdosed patient to the emergency department of the nearest hospital as soon as possible.
Clompiramine may sometimes cause ‘Serotonin syndrome’ which is a fatal syndrome, therefore consult your doctor before taking the drug and do not use this drug when you use other medications, without doctors consultation.
In some instances, such as allergy towards the drug, recent myocardial infarction, arrhythmias, mania, liver diseases, glaucoma and urinary retention this drug is prohibited. Clompiramine is also prohibited during the periods of pregnancy and lactation as it may cause heart defects in unborn and children.
The withdrawal symptoms are likely to occur in the case of long time usage and sudden discontinuation. This drug is strictly given under a prescription, therefore sharing it with others is really unsafe.
