Milnacipran or otherwise called as Ixel, Savella, Dalcipran or Toledomin is an antidepressant. It is a serotonin and norepinephrine reuptake inhibitor. By inhibiting the reuptake of these neurotransmitters, it increases their effect on the brain. This drug is prescribed mainly as a treatment for the pain in Fibromyalgia. But, some other countries other than USA use this to treat depression too.

Just like other medications, Milnacipran too is entitled to have adverse effects. The most common among them are, palpitations, tachycardia, blurred vision, nausea, constipations, vomiting, dry mouth, abdominal pain, chest pain, chills, loss of appetite, increased blood pressure, headache, dizziness, tremors, insomnia, anxiety, dyspnoea and hot flushes.

The normal dose for an adult is 100mg per day and it is given as 50mg two times a day. The beginning of this drug is being done stepwise. Day 1 the patient is given 12.5mg once daily. Day 2 – 3 the patient is given 25mg per day (as 12.5 mg two times a day). Day 4 – 7 a dose of 50mg per day will be administered (as 25mg two times a day) and after the 7th day the normal adult daily dose of 100mg per day can be started. Based on how the patient responds the dose can be increased up to 200mg per day too. In case of any overdose, please rush the patient to the nearest emergency department.

During discontinuation of Milnacipran, withdrawal symptoms are being observed. Therefore abrupt discontinuation is not being done. In case if you want to stop this medication, consult your doctor and he will reduce the dose slowly until total discontinuation, to prevent withdrawal symptoms.

If you missed your tablet, skip this missed dose and take the next one on time. Do not take both the tablets at the same time. Continue to take the medications even if you feel well. Take this pill by mouth with or without food, but taking with food may reduce any gastric irritations.

Milnacipran should be stored in a place where the children cannot reach. This drug does not require any special storage condition. However, the drug should not be used after it has been expired, which means after the last day of the month which has been stated. Moreover, the tablet should not be used, if there is any discoloration and so on.

This drug is contraindicated if:

  • The person is hypersensitive or allergic to Milnacipran or to the other ingredients of the Milnacipran.
  • The person is pregnant, or might become pregnant or a breastfeeding mother.
  • Patients under 15 years of age.
  • Patient has undergone or undergoing concomitant treatment using MAO inhibitors (Phenelzine, Tranylcypromine, digitalis glycosides, etc.)
