What Is This Drug?

Jardiance is an oral tablet for lowering high blood sugar level in type 2 diabetes in adults. It has empagliflozin as its active ingredient. Empagliflozin is a sodium glucose cotransporter 2 (SGLT-2) inhibitor which has a blood glucose–lowering effect. Empagliflozin does not treat diabetes but controls it. It protects against stroke, heart attack and other diabetes-related combination like kidney failure, nerve damage, and eye problems. [1]

Classification and Mechanism of Action

Empagliflozin is used in treating type 2 diabetes, but is not useful in type 1 diabetes or diabetic ketoacidosis. SGLT 2 is a predominant transporter that reabsorbs glucose before it is excreted out from the body through urine. Empagliflozin inhibits SGLT 2, thereby preventing the reabsorption of glucose into the blood circulation and lowering blood sugar level. Empagliflozin thus leads to an increase in glucose excretion through urine. [2]

Looking After Your Drug

Store in a cool place. Protect from moisture, direct sunlight, and heat. Do not freeze. Keep it away from children. Store it at 25°C; variations permitted, 15°C-30°C. [2]

Dosage Form And Strength

Empagliflozin is available in a tablet form in two strengths, 10 mg and 25 mg. [2]

Dosage

Empagliflozin 10 mg tablet should be taken by mouth once a day, in the morning, with or without food. There should be a gap of 24 hours between doses. The dose may be increased to a 25 mg tablet as needed. Take empagliflozin tablet as directed by the physician. Do not take more or fewer doses of empagliflozin than prescribed by your doctor. [2]

How to Take the Medication?

This drug should only be consumed if prescribed by a registered medical practitioner. Empagliflozin should be taken with water, before or after meals. It is not recommended for individuals under 18 years of age. [2]

Overdose

Seek medical help in case of overdose and contact the Poison Control Centre. Removing unabsorbed material from the gastrointestinal tract, supportive treatment, and clinical monitoring is the basic treatment employed for overdose. The exact treatment for overdose depends on the clinical status of the patient. Empagliflozin removal by hemodialysis has not been studied. [2]

Warnings and Precautions

Individuals should not use empagliflozin if they are allergic to it. [2]

Empagliflozin is avoided in pregnant women and breastfeeding mothers. It should not be used in people with renal impairment, end-stage renal disease, or dialysis. It should not be used in people with low systolic blood pressure and patients on diuretic therapy. It must be avoided in individuals below 18 years of age as no data is available on the effects of the drug on that age group. Don’t use any other medication with empagliflozin without consulting your doctor. [2]

Possible Side Effects

Empagliflozin is an oral therapy for the treatment of type 2 diabetes in adult men. It may contain some ingredients which can cause side effects. Before using empagliflozin, individuals should consult a physician. [2] Some of the side effects are:

  • Dehydration which may lead to dizziness, lightheadedness, or weakness.
  • Women on empagliflozin therapy may suffer from vaginal yeast infections and may experience symptoms like vaginal odor, vaginal itching, and white or yellow vaginal discharge.
  • Men undergoing empagliflozin therapy may suffer from balanitis or balanoposthitis which is a yeast infection of the penis. Such individuals may experience symptoms like redness, itching, swelling, or rash of the penis; discharge with a foul odor from the penis; and pain in the skin around the penis. [2]

Consult a physician if any of the side effects of empagliflozin are observed. The physician may prescribe antifungal or other therapy along with empagliflozin to overcome the side effects. Empagliflozin can cause other severe side effects which include ketoacidosis that may lead to nausea, vomiting, tiredness, and abdominal pain; serious urinary tract infections; sudden kidney injury; increased low-density lipoprotein cholesterol; and hypoglycemia when taken with other antidiabetic drugs like sulfonylurea or insulin. The symptoms of hypoglycemia are headache, drowsiness, weakness, irritability, hunger, fast heartbeat, sweating, and dizziness. [2]

When the Drug Should Not Be Used?

Prior to the treatment, the medical history of the patient should be studied thoroughly. The doctor must be informed about any kidney or liver conditions, urinary tract infection or problems with urination, pancreatic conditions, alcoholism, any surgery plans, current or future pregnancy plans, and breastfeeding. The doctor should also be informed about any prescription or over-the-counter medicines taken by the patient, especially if it is a diuretic or other antidiabetic drug therapy. [2]
