Maprotiline is a tetracyclic antidepressant which is also trading under the names of Deprilept, Ludiomil and Psymion. It mainly acts as a strong norepinephrine reuptake inhibitor and weak serotonin and dopamine reuptake inhibitor. There are also other mechanisms of actions such as antagonism of H1 receptors and a1 receptors, leading to the adverse effects of this drug. Pharmacologically this drug also has sedative, anxiolytic and sympathomimetic activities.

Maprotiline shows 66% - 70% bioavailability and 88% protein binding capacity. It is metabolized in the liver and excreted by the kidneys and bile with the half life of 27-58 hours in the blood.

This drug is used to treat all severities and forms of depression, panic disorder, neuropathic pain, bipolar depression, anxiety, tension and insomnia. It can be administered per oral, intramuscularly and also intravenously. Starting from a lower dose such as 75mg per day, it will be gradually increased by 25mg per day, according to the patient’s response towards the drug. But, the daily dose should not exceed more than 150mg. Once the symptoms are controlled, your consultant may reduce the dosage until 50-75mg per day. This tablet is better tolerated when it is administered with a meal. In case you missed a dose, leave it if it is the time for the next dose; do not try to add up to two doses together to compensate the missed one.

There are several side effects which may bother you while you are on this drug; they are Constipation; dizziness; drowsiness; dry mouth; nervousness. In case of overdose of Maprotiline, it is important to take the patient to the emergency department of the nearest hospital as soon as possible.

The withdrawal symptoms are likely to occur in the case of long time usage and sudden discontinuation. This drug is strictly given under a prescription; therefore sharing it with others is really unsafe. There is no known potential for abusing or dependence of this drug.

There may be drug interactions with other group of drugs, therefore tell your doctor all the drugs you have been taking and are taking at the moment before he prescribe you Maprotiline. Hypersensitivity to maprotiline or to other tri-/tetracyclic antidepressants, hypertrophy of the prostate gland with urine hesitancy and closed angle glaucoma are absolute contraindications for this drug, while concomitant treatment with a MAO-inhibitor, serious impairment of liver and kidney function, epilepsy and other conditions that lower the seizure threshold (active brain tumours, alcohol withdrawal, other medications), serious cardiovascular conditions (arrhythmias, heart insufficiency, state after myocardial infarction etc.) and patients under age 18 are relative contraindications.
